Broken step
Things will go wrong. Things will definitely go wrong. You do what you can to fix the problem and come out smiling on the other side. Phew! Now what? In any learning organisation the very next thing is to work out what went wrong so you can make sure it doesn’t happen again (ideally, so you always make no or new mistakes, and don’t keep repeating the old ones over and over).
The immediate issue is to work out whether the problem was a one-off or exposes something systemic. For one-offs, just give the feedback, fix the code, add some coaching and move on. For systemics, fix the system.
The question is defining which is which. Was the broken step possible because of the system or simply a thing on its own? Err towards thinking more and jumping slowly.
